You may save some time by downloading each document above, printing it, and filling
it out. These are standard “initial” intake forms that need to be completed to be accepted
as a new patient. If you are unable to bring these documents we will provide them for you
to fill out prior to seeing your doctor for the first time. After the first visit you will not
be asked to fill out any more intake forms. These forms help us to better help you.
